Notice of Privacy Policies
The law gives you certain privacy rights regarding personal
information and personal health information, hereafter
referred to as PHI. This notices describes how the Semons Financial
Group may use the PHI of you and/or your dependents and your
rights regarding it's use.
Why do we collect your PHI?
The Semons Financial Group collects your PHI in order to assist
your employer in offering you and your dependents quality
benefit programs at the best possible price. A critical part
of that process is having the ability to give prospective
insurers accurate personal information as well as historical
health data. As part of our relationship with your Employer
we will use this information to obtain price quotes from
prospective insurers or other vendors and/or develop recommendations
regarding the benefit programs he may wish to offer you.
We are required by applicable state and federal laws to maintain
the privacy of your PHI and only disclose it as permitted
by law. This notice explains how we will collect your PHI,
how we protect your PHI, to whom we will share your PHI and
your rights regarding the disclosure of your PHI.
How do we collect your PHI?
In most cases we collect your PHI directly through you, your
dependents and insurers.
How do we protect your PHI?
We protect your information by encrypting all data submissions
that occur over the Internet and by maintaining your PHI
in an encrypted state while in our data base. If you have
followed our recommendation regarding customizing your password
for access to our systems, these levels of security will
protect your PHI well beyond federal and state requirements.
Additionally, all employees of the Semons Financial Group
are under obligation to hold confidential all client PHI
and are subject to disiplinary action if they fail to follow
this code. At no point, now or in the future, will we sell
any of your PHI to any outside vendor.
Who do we share your PHI with?
We will never disclose your PHI with any institution, vendor,
or individual except for uses approved by law and/or the
following uses:
Payment Activities: We may use and disclose your PHI to fulfill
payment activities which may arise from services offered through
your employer for your benefit.
Plan Sponsors: We may disclose some elements of your PHI to
the plan sponsors which is necessary for them to carry out
their general administrative and fiduciary activities.
Quote Submissions: We may disclose your PHI to secure quotes
for coverages or services that your employer would request
or which, in our opinion, may be beneficial for you and/or
your employer.
Service Requests: We may disclose your PHI to you, your dependents
and in limited cases your friends and/or family members, if
requested, and if we determine that the disclosure of said
information will be in your best interest.
Required by Law: We will disclose your PHI when we are required
by law, including court ordered proceedings.
What are your rights regarding the disclosure of your PHI?
You have the right to review and receive a copy of your PHI.
This does not include the right to receive information that
may have been generated or obtained through the use of your
PHI. You have the right to request an amendment to your PHI
if you believe it to be inaccurate. You have the right to
request that we put additional restrictions on our use or
disclosure of your PHI. You have the right to request that
we communicate with you in confidence about your PHI by alternative
means provided you specify a reasonable alternative means.
